Hiding fields on NewForm.aspx and EditForm.aspx

First open entry form (NewForm.aspx / EditForm.aspx / DispForm.aspx) in advanced mode in sharepoint designer. Then search for tag :

<asp:Content ContentPlaceHolderId="PlaceHolderAdditionalPageHead" runat="server">

Now copy & paste this JavaScript code after this tag :
